Abstract Background Unprovoked spontaneous uterine rupture in a primigravid unscarred uterus is rare but serious obstetric complication. Our case highlights prelabour primiparous patient with an anterior placenta praevia and transverse lie at 32 weeks. Case presentation The presented severe continuous abdominal pain abnormal Cardiotocograph antenatally. An emergency Caesarean section done suspicion of concealed abruption revealed cornual 2 L hemoperitoneum. Conclusion her baby recovered well from surgery, thus, emphasizing the necessity timely intervention. association previous laparoscopic treatment endometriosis adenomyosis explored.
